Advance Care Planning in the Digital Age
Advance care planning enables individuals to express their preferences for future health care and treatment, making their wishes known if there comes a time when they cannot speak for themselves.

Storing advance care planning documents in My Health Record means patients and healthcare providers involved in their care have secure, rapid access to these important documents whenever they’re needed.

This session will highlight how advance care plans and goals of care documents may be shared via My Health Record, along with a live demonstration of how to access and view this information.

There will also be a discussion of medio-legal issues associated with advance care planning documents.

Presenter: Dr Kathy Rainbird

Dr Kathy Rainbird holds a PhD in Behavioral Science in relation to Medicine and is a Senior Provider Adoption Lead at the Australian Digital Health Agency.
Kathy is based in Western Australia and has been working with indigenous medical services and other healthcare providers both across WA and nationally, training them in the benefits, features and clinical use of My Health Record.
